Jose Mourinho has been announced as the Premier League Manager of the Year, while Eden Hazard was named Player of the Year.
Mourinho guided Chelsea to their fifth title, while Hazard played a starring role for the Blues, as they were confirmed as champions with three games left.
It is the third time Mourinho will clinch the award, after winning it in the 2004/2005 and 2005/2006 seasons.
For Hazard, it is his third prize this term, after taking home the PFA Player of the Year and Football Writers’ Association Footballer of the Year.
The Belgian scored the goal that sealed Chelsea’s title success against Crystal Palace and netted 14 Premier League goals in total.
Mourinho, who never won the Manager of the Month award during the season, beat fellow nominees Ronald Koeman, Garry Monk, Nigel Pearson and Arsene Wenger to the overall award, while Hazard defeated fellow Blues stars John Terry, Nemanja Matic and Cesc Fabregas.
